Team Senior™ offers in-person long-term care guidance to seniors and their families. Our service is 100% FREE to your clients! We are compensated by the facilities that we refer to. We can refer all individuals except those who receive Medicaid (because of the Federal Anti-Kickback law).
Our team consists of nurses, long-term care experts, senior advocates, educational staff and more. We love what we do and it’s reflected in our service and reputation!
Why Refer To Us?
• We personally get to know the facilities that we refer to – visiting each of them, performing an assessment, and cataloging what they are capable of and what they prefer.
• Prior to making any referrals, we perform in-person assessments with our clients – thoroughly assessing what their care needs and social needs are before making any recommendations.
• When requested, we tour facilities with our clients.
• We perform follow ups with all of our placements, ensuring a smooth transition.
• We are a local business and we have our finger on the pulse of the Southern Oregon senior community.

Assistance is provided for long-term care housing, in-home care, home health, hospital discharges when transitioning to long-term care, coordination with doctors, pharmacists and caregivers, initiating care plans & PRN parameters (provided by a primary care doctor, resident nurse or the discharging physician).
